To ensure that everyone is seen in a timely manner, we ask that you arrive for your appointment 15 minutes before your scheduled appointment time.  If more than one child is being seen, you may be asked to arrive even earlier.  Late arrivals may be asked to wait or to reschedule depending on how full the day's schedule is.

Please bring the following to your first appointment:

• Records from previous physicians if available (especially growth charts)
• Immunization cards and records
• Any prescribed medications child is taking or has taken in the past
• Insurance cards and information
• Forms of payment
  
• Power of Attorney, if the patient's legal guardian will not be present

Filling out complex medical forms annoys us, too.  So when you come to our office, you will be asked to fill out two very simple single-sided forms (a medical release form and insurance information).  This will take you all of about 3 minutes, so you don't need to fill out anything beforehand.  Our office uses electronic medical records, so once we enter this information into our system, the papers are shredded and recycled.  Our goal is to be as efficient and environmentally friendly as possible.
